Because it's a Damage Over Time passive. The same kind of ability that activates Ghulk's face me, also similar to Dormmamu's degen and Domino's critical failure.

Fisticutts doesnt interact with passive abilities (it would break something in the game if it did).

A confirmation of what it is, and was denying the existence of "passive buffs/debuffs" in the game. It's not un-constructive for sure, but thats personal.

I actually liked that you mentioned fisticuffs interacting with passives would kinda break things. The whole point of passives is to be a little more impervious to champions abilities and nodes that might take them away or change them. That was a useful addition IMO.
