This has been a known issue for a while. Happens a lot in variant 3. Don’t know what they’re doing about it.

Did u also have the Mandela Effect where u were under the impression that Groot was as good as he was in GoTG1? 😛

It said they made further improvements to prevent it, they didn’t fix it though.

I only experienced the bug in Variant 3 with sentinel, i died a couple of times but was stacked on revives.

There's a thread about this issue somewhere. It happened to me w Omega Red multiple times, but no solution has been announced yet.
