Heavy hits are not activating some of the time

I am having an issue where I will start a heavy attack and when I release it to so the champion throws the heavy it is stalling in the stance (like it is charging) and does not throw the heavy. This does not happen all the time. Maybe around 30-40% of the time. I don't use heavy often as a part of my game play but I opened a 5* LongShot (LS), awakened him, and put some sig stones into him and took him to R5. I use (or try to use) my heavy to change good karma from LS to bad karma on the opponent and this problem seems to be happening quite often. Has anyone had this happen to them or is this a known issue?
