I died to this bug. It’s like fight domino but I fought a Corvus. I died and gave it a glance charge and after that this bug happened. Has this happened to anyone

Please read up corvus's ability! After getting a tech charge, corvus reduces tech champs's ability accuracy by a flat 25%. Which means ghost has a chance for her phase ability to fail!

the phasing worked just fine, don’t see a problem

If corvus killed a tech champ and gained a charge from it he can then reduce tech champs ability accuracy by 25%. Not to mention that since you are fighting in AW the opponents may also have assassin mastery which also reduces ability accuracy.
