Some kind of bug i guess.

I just achieved paragon title and before that title i already explored threat level 5 in side quest before paragon and now i just got a new mission and i tried to explore again but i got nothing nothing. Is there any way to complete that mission or this is some kind of bug
Guys help me with this.
