Are revives just gone forever? I looked in every single classic variant and I found one....and it went away after a run. They used to be all over the place

Another revive spot is 6.4.6 grandmaster quest, which will unlikely be never nerfed, but good luck grinding there lol.
But I'll say I found most recent success in 7.3.6, full revive farm cycle there.
Some say 6.4.6 but that's a lie because you'll have to face grandmaster and that's not worth the effort. 😞
7.3.6 is new farm zone, try it.
if you are looking to build revives for content like necro, just take your time and do it slowly.

Sounds like you’re just “peeking in”, and if no revive then you’re moving along to peek in somewhere else.
You need to complete a quest for that quest to respawn its items (and it's just for “that” quest).
So otherwise, you end up with all of them being “dead quests” (aka no revives), because you’ve run each one until a time when there was no revive on it's last respawn (and haven’t run them again, since there was no revives in them), and eventually got all of them into a “dead” condition.
(**as to the “across the 200+ chapters/paths that are in the game”)
even at 2 per day that is still 28 revs.
the only limited thing you mentioned is SOS which is fully soloable for all objectives.
and also has 2 weeks for a single fight.
secondly you are the one that asked for 5-6 a day.
we can all acknowledge there is 2 available already and you asked for 3-4 more in regular quests.

I'll bet there is 3-4 revives somewhere right now on repeatable maps. But if you want more tomorrow, you'll have to do them all to regenerate them.
Asking for 3-4 to spawn a day in a map that itself resets daily? Yes, that is too much to ask for. Revives will keep for 14 days in stash. Without Sigil, the L1 and L2 inventory cap is 20. So if you could farm four L1 and four L2 revives per day, that would allow a player to save up 152 revives. Even if you could only farm four of one of those revive types, that would still be 110 revives (because you'd have 20 inventory of L1 and L2, plus 4*14=56 of the farmable one, plus 14 of the non-farmable one that you could still get one per day using conventional means).
For all intents and purposes, this is essentially an unlimited number of revives for most end game players for most content in the game. The current non-Sigil cap is 68 which is still a lot, before accounting for things like EQ completion offers or store offers for units that are not all that expensive.
