is this for the Lady Deathstrike fight? She has a node that puts a passive disorient on you reducing your ability accuracy. So that could be failing your armor break.
If its not that fight, post the nodes of the fight or we won't be able to help much.

Disorient only affects defensive ability accuracy, so has no effect on his armor break.

The culprit is that lady deathstrike is immune to armor break while her nanobot regenerator is active, which applies during your special attack hits against her.
