Basic dailies: missing catalysts from chests

This has happened to me multiple times. I have had it happen with t2 and t3 catalyst.
After clearing the daily, if you replay it and take the hard path and receive a catalyst from the chest, sometimes it will not go to your inventory. It's like you never got one.
Dailies are a pain and are annoying because you just auto fight your way through. But, not receiving your catalyst just makes it that much more frustrating.
