Champion Availability Per Tier

I'm an intermediate player and I would just like to know if there's a list of champions available per tier (like if some champs are only available as 1* - 4* or if they are also introduced as 5*).

Has it ever been explained why some of these characters never got 5*? I guess I could understand Wolverine; that regen would be nuts, but why others?
