Side Quest Still Bugged?

How am I supposed to access the tier 6?, I have done tier 2 already still nothing, and I have gotten tier 6 casket already but still nothing

Each time you buy a casket (ANY casket), your available pair of quests gets bumped up by 1 difficulty.
