Captain Britain bug - doesn't apply debuffs correctly

Mobile Carrier: Aldimobile (Optus)

Game Version Installed: 36.0.1 - also 36.3.1

Champions Affected: Captain Britain, 6r1, sig 20 - also 6r2, sig 20

Description of the Issue: Have experienced this since first gaining the champ. Whilst in psionic mode, debuffs do not stack as intended and am only able to place one weakness and one exhaustion, when her description states that she can place max 4 of each debuff. The slow debuff also does not refresh, when it is stated to be non-stacking and so should instead be re-inflicted each time a second medium is performed. It seems that debuffs can't be placed at all if the debuff is already active.
This is mostly a copy-paste of my post from July 25. In the meantime, this bug has also been noticed by @Strategic @TripleB and @issamaf80 , and I'm sure that they can provide more info if needed.
