How does despair work?

Upon first glance, many people would assume that with despair maxed out, 20 ruptures on the opponent would stop or reverse their healing.
The description reads: Debuffs applied to enemies reduce their healing and regeneration effects by num% each. Up to a max of 100%.
So a member of my alliance dived into war, node 3, which was a serpent, and tried spiral against him. He stacked up his debuffs but he would never stop serpent's healing and he would never gain charges.
Why is this the case? Is it because it's a damaging debuff? Is it because its only 1 type of debuff? Any help or clarification is appreciated.

In war, every champ would have the recovery Mastery which gives an additional 15% regen.
So Serpant still has 15% regen. And when we're talking about millions of health points, it is significant
