Cable's defensive abilities not triggering

Lately I've noticed that my 4* 4/40 Cable's defensive abilities are not triggering properly. I can't remember the last time regen procced while in a quest (works fine in arena), but, you know, 15% chance, so maybe I'm just unlucky I thought. But just now I was in arena and a Black Panther (not CW) inflicted bleed and it didn't trigger degen on the BP like it's supposed to. It's supposed to have a 100% chance, and I can't think of any reason Black Panther or a player's masteries would stop the degen from triggering. I went into a duel and tested it against another bleed opponent and it worked fine, so I'm not sure what's going on here.

Just remember it was a Kang's team matchup, so masteries shouldn't have been an issue in any way.
