Question- Aspect of Nightmares buff.

If the fight finished befor 12 sec. and I lost health ... what will happen , does it mean I have to fight until I gain my health back.

yup. it is pretty much what you said. fight or wait out the 12 sec timer before landong the last blow

You need to start the fight above 50% hp. Or somehow be able to get that during those 12 seconds.
Otherwise you'll lose due to the damage taken
