I think the most likely explanation, if you are taking damage from caltrops bleeds, is that the vicious is applying to self-inflicted bleeds, and is counting caltrops. so the potency is 100% base - 1…3 -

It’s theoretically possible? It would be difficult to hit Valiant without any 6* above rank 3, (and you get enough gems in Act 8 that I think you could get Paragon just using gems).1 -
