Fatigue with OG Ultron in EQ 2.2

As stated in the description of this boss’ debuffs node, he is supposed to apply a fatigue debuff every time he successfully evades an attack passively.
I found that he applies this debuff even when he doesn’t evade.
Is it normal?

If you can’t see the video, I’m using Doc Octopus, I’m only parrying and using heavy attack. He doesn’t evade once and still applies the fatigue debuff.
