Unable to see Level 4 Permission Slip

Unable to see Level 4 Permission slip in the Souvenir tab in the store, I can only see up to Level 3. Is this supposed to be the case? Anyone else having this issue?

    have you already completed Act 4? Level 4 Permission slips are accessible only to those that have beaten Maestro at least one time :)
