Hits not connecting [More Info Needed]

I was just fighting a Phoenix(she wasn't in her phenoix mode where she regens) and I was attacking her but some of them did not connect. Has anyone else faced this issue.

I was using quake and was currently doing the sinister foes event quest.

Also it wasn't every hit, think it was just the 5th hit in a combo.
