Medium Attacks Activating at Random

In another thread, that has been buried, I brought up an issue with medium attacks randomly activating during L, L, L, L combos.
Try using Hulk in Alliance Quest. He is almost unusable because almost EVERY combo attempt is a medium attack on the second hit, which obviously ENDS the combo and forces you to dash back or parry and start a new combo.
Again, this is on iPhone 7, most current iOS.
