Book 1 Act 4.2.6 Double or nothing bug

So I’ve been going through Act 4 to 100% it and for some reason, the buffs sometimes fail to apply. They never even showed up for a split second. Is it supposed to be effected by ability accuracy reduction or no?

    Thing is this seems to happen randomly. Sometimes all are applied then the next they don’t
