Champion fight advice

I’m still struggling to handle the last 10% of the champion boss in 6.2.6. Below I’ve included a link to the video I shot of it. Watching the video it looks like I should have gotten all 5 of the needed charges, but they didn’t trigger. Is something wrong with my timing or spacing for the dexterity?
