Weird Glitches in game

Has anyone else been having weird glitches in game?

One weird glitch I have is Warlock’s head will disappear sometimes after SP1

Almost in every fight one of my character freezes during the fight which makes me lose health as I get confused

I'm standing about a foot in rubble in many fights as of today.
