Only if there was an actual bug somewhere that allowed the Revive (or Heal, or Energy) item to respawn itself WITHOUT actually finishing the quest (from Quitting and coming back in again)
Which I don’t think there is any current case like that.

I farmed two from act 4 today, one in 4.4.2 and one in 4.3.2 or 4.3.3 I don't remember but it is circumstantial, sometimes it will be there and sometimes it won't be there
