How do I send a help ticket?

For some reason when I gifted 15 lvl 5 potions it went in twice. I claimed one set of 15 but I want to get my units back for the other 15 and haven't not claimed them. Since I forgot how to send a help ticket can someone help?

Choose MCOC -> Select "Submit a Request" -> Choose "I need help" -> Fill in the details about the problem and submit.
