    If i had to guess what's happening, after you throw your firs rmedium , there's a certain window that needs to pass before you can throw first medium attack again. Here, you went in with first medium, got autoblocked(?) And then yoy swiped which resulted in the second medium since not enough time had passed for you to be able to throw first medium again
