Is Quake Bugged?

Quake isn't evading her first hit. At first I thought maybe it's my mistake but then it happened the same while I was attacking. First it happened with Bishop then it happened with Scarlet Witch sigil. Is anyone else facing the same thing?

    Nodes? What quest/mode is it in?

    Bishop has a short range hit, sometimes the distance is not enough and Quake doesn't evade Bishop.

    This^^^
    threw me off the first time I tried in EoP but once you realize it it’s easy to get used too
