Completed paths partially unexplored?

I’m currently in the process of exploring act 4 and act 5. There has been a couple quests that I’ve done paths on but there’s one little section that says it’s unexplored? I have to complete that path again to fully explore the quests.

Anybody else having this issue? Are there any fixes?
