SQ progress reset?

I thought we can get a 6 star AG if we fully explore threat level 5. But my progress is reset everytime I enter. I can't remember all these paths, it's like 40 of them. Is this a bug?

Yes it's a bug and there is a safe not dropping bug that is filtering some players from moving past zone 1
