Does anyone know if there is a bug with completing the explore 100% episode 1, 2, 3, 4??? I have completed week 1 and 2 on threat 1, 2 and 3 but threat 3 stating that I have only completed 1 and not 2

You didn’t get the objective for TL 3 until AFTER you had already completed one week's worth while still Uncollected.
After that is when you must have become CAV, and done week 2, and so it doesn’t reward newly available objectives retroactively after you advance to new progressions.

Just like your Calendar doesn’t upgrade to next progression level of stuff until the whole month rolls over.

Try submitting a Support Ticket, they seem to be giving into these type of requests.
