Threat level side quest rewards issue

Unable to claim my rewards for 100% exploring threat level 4. Don’t know whether gaining thronebreaker has something to do with it.

Same. Unable to claim rewards for Threat Level 5 after 100%. Game only says I completed 3 of 4 at 100. WTF
