Heimdall Synergy not working?

I have a 5* Unawakened Heimdall which I use when doing questing for his 2 second "last chance" synergy which stops the knockout hit from knocking you out.

However I've noticed at times it just does not work. Like the lights appear and the sigil appears at your feet yet not even a second passes and you're dead.

The unstoppable/unblockable? (Yellow and purple) effects don't even appear below the health bar and the champ just gets knocked out.

I'm assuming this is a bug or I'm just missing something

    Some champs can prevent the activation. Aar , nullify, stagger, all those work against it. And necropolis also has a counter for it

    So let's say I used Chee'Ilth against the Medusa boss in "The World Of Today" what would stop the activation?

    That would be buffet. Doesn’t stop the activation, but nullifies the buffs
