Side Quest Threat Lv. 5 Locked

I want to do level 5 of til deathless do us part, but it says it's locked, with the message "unlock by completing the preceding quest".
I assumed to mean I needed to do lv. 4 first, but after completing it I still get the same lock and message.

Why is this, and is there something else I need to do to unlock the quest?

This month you only get one level, based on progression. The message that you can unlock other levels is inaccurate.
