2nd M not working

Just doing some AQ fights and my 2nd medium hit was not inputting. My guy would just stand there after 4 hits. Happened several times in the 2 fights I did.
I restarted my game and did some duels and still having the same issue. It only seems like I cant finish a 5 hit combo ending in a M. But doing 4 hit combo ending in a M seems fine.
