Crucible Black Widow Scytails phase 3 not working?

After I died and revived during the BW fight in act 8 crucible, when she was at 11% health (phase 3), her sp1 doesn't set a trap, and neither does her sp2. I've tried reviving and replaying the fight, but nothing seems to work.

This seems to be a problem that has been around for quite a while, judging by this reddit post 2 years ago

I would rather not have to restart the crucible just to fix this bug :(
