Random KO’s

Is anyone experiencing random ko’s during fights? Whether they’re arena battles or in the quests? I’ve noticed this a few times and it just happened to me again. I was playing arena and my 3* punisher was ko’ed with 54% health against the Red Hulk. His incineration effect happened and no other hits were received. I’m just curious if anyone has encountered this as well with random ko’s or general glitches that characters don’t have?
