Attacks not connecting

Noticed this with Gwenpool and Medusa. My first dash attack just phased through and didn’t connect. Has his happened to anyone else?

I had the same issue today with Ghost Rider against Sentinels in both AQ and AW. The attack would just pass through them, then they would dash back. It even set off Dexterity when it did this in AW.
