Not completing 5hit combo

I dont know if its just me but its been happening quite abit lately. My champs sometimes dont seem to finish the combo but instead just stand there and get hit in the face.

I am now having the issue where they dash back during the combo or decide to heavy attack...

Also saw this during my act 5.4 run. It is very random but you would notice they wont finish the combo.
