Was literally about to Ask this. Is it a voluntary add that people get for participation on the forums? Is it a warning by moderators that they're on thin ice? I thought I was seeing posts by people who have since been kicked/temp banned.

Bars on the profile picture means the forum poster is in "forum jail" due to violations of the forum rules that aren't severe enough to get them permanently banned, but have placed them in a kind of probation. I think it applies throttles on their posting, and I think it also prevents them from starting new threads until the probation expires.

Permanently banned forum posters do not have bars on their profile picture. Instead, their profile picture is completely replaced with one of these:

Jail is temporary. That is permanent.

Why do some players profile pics have a bar before the picture?

I've had bars multiple times because Forum mods failed to see the irony in my comment and took it at face value. It's been awhile. If you commit an infraction while jailed you can be banned, or if your post/comment warrants it you can go straight to being banned.
