Hylerion in aw killed my gwenpool sig 200 with sp3 bug

Bug in aw: hyperion was on left side boss nod (power gain and unblock sp2) and he killed with sp3 my 5* gwenpool with sig lvl 200 (on first sp3) how is it possible? Sry for my english

If you are under 18% health (I think that's the border)
And enemy has assassin's mastery those abillities can fail. That's the most likely thing that happened here
