Human Torch Prefight not triggering

Two war I have gone in with Torches prefight activated and both times I lose the charge but have no nova?

It has cost me a death both times and both times the defender is left with less than 15% health the nova would have killed 100%

    Had same happen on a rintrah boss, activated but didn’t work
