Side quest is 100% but not getting items

I have got all of the threat level 2 side quests to 100% but on the quests page it only says i have 3/4 of them done to get the rewards for completing all 100% not sure what to do as have replayed all of them

    same thing happened to me with threat level 3

    Yea same thing happened to me grinder so hard but not able to get the reward :(
