Having to Repeat Fights

in Bugs and Known Issues
After I win a fight in any quest, the game makes me repeat that same fight sometimes. For example, I beat Sam Wilson in the EQ with Warlock and after the victory screen disappeared, it made me redo that same fight. This has been happening all week and is getting annoying real quick. Is this a known bug?

    Same. Has happened to me right now againt IMIW in 3.1 theonebreaker difficulty, 3 times in a row now, ffs

    happened right now, i cant revive champs. annoying
