RoL Wolverine came back from the dead?

I was just randomly testing Kitty Pride with a full synergy team in RoL as one does when they're bored and had gotten to wolverine. Knowing that Kitty wouldn't be able to out damage his regeneration without it taking 15 minutes, I used Magneto instead. However, I was basically not paying attention since it was so easy so I see my SP3 kill him and then I just mindlessly go to the next fight. And yet, I look down at my phone and see Wolverine again? I'm not sure how this happened since I killed him.

    Remove any piercings or metal jewellery and try the fight again

    It also happened to me the other day, i fought Dust with Titania in the sidequest. I've won and then, after the loading screen, she was still alive ????
