How do you get this pic? I've seen a few--on players ranging from 1,200,000 to 250,000 total rating--but it doesn't seem available to me.

    Because you probably already sold your Summoned Symbioid. He still hasn't run away if you didn't sell, which allows you to use him as a profile picture.
