Something weird happened when I got a dupe 3 star today

Did the chilled challenge and got 3 star shards to get a crystal, duped Storm and when the usual stuff was going up of the reward signature levels, class Iso, gold and 4 star shards, I then had a blank square appear with 'bonus item found' above it, but there was no item in the square or on the rewards after it completed.

Anyone else getting anything like this?
