War machine in UC 3.1

in Bugs and Known Issues
I was using ghost with wasp synergy in this fight. When I phased to use her l2, he dashed back. Ok, whatever. Her 2nd hit landed. Then he dashed back again and her last hit of her l2 missed and he punished me. How does this occur? He should have been disabled after her 2nd hit landed.
