Ghost bug-opponents is able to attach while I am phasing

This is in quest 6.2.5. I am taking my ghost against proxima and wasp. I noticed I was attacked by proxima while I am phasing but I did not record it. This happened again with wasp. Also during the final Mordo fight, I dashed back and then trying to launch a sp2 attack but the special attack did not launch and I remained phasing
