Help identifying an offer

A while ago, I purchased an offer where you got a series of crystals based on your account level.
You got premiums at low levels, 1 or 2 3,*s, a 4* (which was useful at the time) and a 5* but cant remember if I get that at summoner level 50 or 60.
It doesnt appear in my cards so I cant check.
Which level do I get this 5* crystal at? At level 49, so hoping it's at 50.
Any help remembering would be appreciated.
