Heavy attack triggering during aq

I've done everything I can to fix this on my end including buying a new phone. Something is causing me to use heavy attacks during aq when I haven't launched them. Is this a known issue of any kind??

    I just spent $200 on a new phone and $50 on new masteries hoping this issue would resolve itself somehow and it's still happening. I have a video and I'm sure I can get more. It just happened in aw too
