If you don't chase things in this game every content will become super easy one day. But we often come to clear it when our rosters are not strong enough.
When it comes to niche matchups in acts 4 and 5, I remember venom in 4.3.6, juggernaut in act 4 with like 130k health and 10k attack! In act 5, there's only about 3 fights in the whole of it that had higher stats than that juggy! I remember Elektra with mesmerise, the weird cross section path and some other BS... Act 5, there was a lot more nicheness and overall BS, that frickin caltrops path in 5.2.4 with masochism! There was an electro, a juggernaut and a magneto. There was the collector of course, still one of the most annoying fights in the game, which of course, a lot of people would leave that quest for last when exploring, me included. The entire 5.3 is a BS fiesta! 2 spiked armor paths, two! I also remember the dormammu in 5.3.4 with 80% physical resist, and in the cosmic quest! 5.4 wasn't overly insane, Ant-Man with all or nothing and cornered, pretty bad. That modok of 5.4.4 with his massive healthpool, easy solo for aegon either way. Outside of those, I'd say 5.4 was manageable enough.
